Tirzepatide: the biggest numbers among approved options
Tirzepatide acts on two receptors, GIP and GLP-1, and currently leads on raw weight loss among approved drugs. In SURMOUNT-1, adults with obesity lost an average of 15.0% of body weight on the 5 mg dose, 19.5% on 10 mg, and 20.9% on 15 mg, over 72 weeks, compared with 3.1% on placebo. More than half the people on the higher dose lost at least 20% of their body weight [1].
That’s a large, durable, placebo-controlled result. Like semaglutide, it’s a prescription medicine with a defined safety profile, not something to take up casually. Between the two, tirzepatide edges ahead on magnitude, but both sit firmly at the top of the evidence pile.

Retatrutide: remarkable data, no approval yet
Retatrutide adds a third mechanism, glucagon-receptor activity, on top of the other two, and its trial numbers are the most striking in this entire field. A Phase 2 study reported an average 24.2% reduction in body weight at 48 weeks on the 12 mg dose [2]. The Phase 3 TRIUMPH-1 results, released in May 2026, reported an average 28.3% reduction at 80 weeks versus 2.2% on placebo, with 45.3% of participants losing at least 30% of their body weight [3].
Those numbers are genuinely remarkable. But “the best numbers in the trial data” and “something you can safely act on today” aren’t the same thing. Retatrutide is still investigational, not an FDA-approved finished product, and the FDA named it directly in a 2026 warning letter to a seller marketing it outside that approval process [11]. The data are excellent. The regulatory status isn’t there yet. Anyone encountering retatrutide right now should be doing so through a clinician who can explain exactly where it stands, not a checkout page.
AOD-9604: the trial that mattered didn’t work
This is the compound where the marketing and the evidence quietly disagree, and it’s worth telling plainly. AOD-9604 is a fragment of growth hormone, sold heavily as a fat-loss peptide. An early, small study hinted at modest weight loss. But the larger trial designed to actually confirm that, over 24 weeks, didn’t: development as an obesity drug was discontinued after it failed to beat placebo. What does exist in the published human record is a safety study, and on that narrow point it looked reassuring, described as well tolerated with no negative effect on glucose metabolism or IGF-1 [5]. Being safe and being effective aren’t the same finding. AOD-9604 has one without the other.
5-Amino-1MQ: promising in mice, untested in people
Technically not even a peptide, this NNMT inhibitor is sold for fat loss based on rodent research. In diet-induced obese mice, blocking NNMT with 5-Amino-1MQ reduced body weight and fat mass [6]. That’s a legitimate reason for scientists to keep studying it. It isn’t evidence that it works in a person, because no completed human efficacy trial has shown it causes weight loss. Buying it for that purpose means betting on a mouse result.
MOTS-c: your body already makes this one
MOTS-c is a mitochondrial-derived peptide marketed as an “exercise mimetic.” Most of the human data on it is observational: exercise itself raises a person’s own MOTS-c levels [7], which is interesting physiology and not much more for anyone thinking of buying it. There’s no randomized trial showing that injecting MOTS-c causes weight loss. The “exercise in a vial” framing runs well ahead of what’s actually been shown.
Tesofensine: real data, but a different kind of risk
Tesofensine turns up on peptide lists where it doesn’t really belong, because it isn’t a peptide at all, it’s a triple monoamine reuptake inhibitor, closer to a stimulant. It does have human weight-loss data: a 24-week Phase 2 trial found roughly twice the weight loss of the approved drugs of its era [4]. It was never approved as an obesity treatment, though, and stimulant-type appetite suppressants bring their own cardiovascular and tolerability concerns, including a faster heart rate. More evidence than the unproven peptides above, yes, but a different risk conversation entirely.

Why evidence and access turn out to be separate problems
